This form is for new entrants who have gained admission into a university.
Applicants must be indigenes of Ukawu, Onicha Local Government Area, Ebonyi State, who are academically sound and indigent.
Applicants must not be more than 25 years old at the time of application.
All sections must be completed accurately and honestly. False information will lead to disqualification.
Attach clear copies of all supporting documents.
Submit the completed application before the deadline — September 20, 2026.

What happens next — complete these steps before September 20, 2026

Download and prepare the paper forms.
Download the Application Form (PDF) and the Reference Form (PDF). Fill both completely and accurately, have the Reference Form signed by your Community Leader or Ward Councillor, and attach photocopies of all the documents you uploaded online.
Submit the pack to your community leader — the completed application form, the signed reference form, and all photocopies.
Send a confirmation SMS to your village coordinator in the format [Your Full Name] - [Course of Study] (for example: John Doe - Computer Science).
